$150/hr for self pay clients.
I require a valid credit card to be on file and autopay.

Insurance

I accept the following insurance In AZ only: Aetna, United Healthcare, Optum, Oscar, United Healthcare Community plan (AHCCCS).

I now accept these insurance plans in CO: Aetna, United Healthcare, Oscar, Optum, Oxford, and Select Health.

Payment

All major credit cards accepted for payment.

Cancellation Policy

If you do not show up for or cancel your scheduled therapy appointment without notifying us at least 24 hours in advance, you will be required to pay a $75 fee.
